We are thrilled you have chosen us as your health care provider, and we welcome you to our office! Our goal is to provide the highest quality care for all of our patients in a timely and respectful manner.
A few things to note:
You will need to bring your insurance card and a photo ID with you for each appointment. If you are unable to provide us with your insurance card, you will be asked to pay for services rendered (we will refund you upon timely receipt of your insurance information or you may reschedule your appointment. You will be asked to fill out up-dated demographic, HIPPA and medical questionnaire forms periodically to ensure we have your most up-dated information. Please be sure to inform our staff if when any of your personal or insurance information changes.
All co-pays and past due balances are expected at time of service, unless a prior agreement has been made with our billing department.
We ask that you allow plenty of time to get to the office for your appointment. We will strive to stay on time. We frequently work in patient emergencies which sometimes causes us to run late. We do our best to avoid any prolonged wait times and at the same time accommodate all our emergency patients.
Please bring a list of all of your prescription and over-the-counter medications with you at each visit.
We understand life happens, and things might come up prohibiting you from attending your appointment. A 25.00 no show/ cancellation fee will be charged for all appointments not cancelled within 24 hours of your appointment time. We keep a cancellation list and call patients on a daily basis to fill cancelled appointments. If we do not have a 24 hour notice or you are a no show, this appointment cannot be rescheduled.
For a shorter time in the waiting room, please fill out the New Patient Forms and email it to us before your visit.
